PharmacistPosition:

• ThePharmacist works in collaboration with other providers to delivercomprehensive medication management that optimizes patientoutcomes
  
• Identifiesmedication-related problems such as appropriateness of drug, dose,route, frequency,duration
  
• Pharmacist must monitorpatient response to therapy
  
• Providesaccurate and adequate drug information to other multi-disciplinaryteam members and drug educational information to patients and theirfamilies
  
• Pharmacist provides supportin other areas of possible need (inpatient pharmacy, outpatientinfusion, oncology, dialysis, specialty outpatient clinics, andsurgery)

PharmacistQualifications:

• Bachelorof Science or Masters/Doctorate Degree inPharmacy
