We are seeking a Care Manager,Social Work for a Hospital and Healthcare company that providesin-home nephrology, primary care, and benefit management servicesfor individual with chronic kidney and end-stage renal disease inBaltimore,

Thisposition will cover a two-hour travel
Shift: MondayFriday 8am However, there could be exceptions where a patient doesrequest a visit after

KeyResponsibilities:

• Raredomestic travel may be required to Nashville,TN
  
• Self-starterwith the ability to work independently with minimalsupervision
  
• Abilityto show empathy and quickly build relationships with patients andlocalCBOs
  
• Abilityto visit patients occasionally or take calls remotely on somenights andweekends
  
• Excellentverbal communication skills both in person and on thephone
  
• Beable to work with Microsoft Office and mobile phone and web-basedapplications
  
• Performin-home care management visits to assess and impact social andbehavioral status
  
• Work closely withCare Team to ensure continual progress on all care managementgoals
  
• Assess social determinants ofhealth needs and develop a plan for addressingthem
  
• Perform behavioral,environmental and social support assessments and surveys asneeded
  
• Deliver individual, familyand group education on living with chronicillness
  
• Engage family and socialsupport groups in the education and care ofpatients
  
• Assess patients and referto behavioral health specialists if diagnosis and treatmentneeded
  
• Help patients to understand,accept and follow medical and lifestylerecommendations
  
• Serve as the pointof contact for patient questions regarding social andbehavioral
  
• Facilitate conversationsaround and consideration of proactive care decisions, especiallyrelating to transplantation, home modalities and AV fistulaplacement
  
• Initiate patientrelationships through enrollment and onboardingprocesses
  
• Review and documentpatient updates and progress in care managementplatform
  
• Identify, vet and buildrelationships with local Community-BasedOrganizations
  
• Introduce patients toappropriate resources and act as the patientadvocate
  
• Serve as subject matterexpert on social determinants for other members of the CareTeam
  

Requirements

Education:

• Masters Degree in Social Work, behavioral sciences or another relatedfield
  
• Currentlylicensed as a LCSW or LMSW in the State of MD
  

Required:

• 2+years previous experience working in care management and/or withchronic illness within a medical environment home health, dialysis,hospice
  
• Abilityto take calls remotely on some nights and weekends
  
• Self-starterwith the ability to work independently with minimalsupervision
  
• Wantto work for a startup
  
• Workswell with ambiguity, drive time, tele-healthcomponents
