PrimaryCare Physician - Responsibilities:

• Evaluatesand treat center patients in accordance with standards of care
  
• Assistsin the coordination of patient services, including but not limitedto specialty referrals when appropriate, hospital and SNFcoordination, durable medical equipment, and home health care
  
• Actsas an active participant and key source of medical expertise withthe care team through daily huddles
  
• HelpsLead Physician and Center Administrator in setting a tone ofcooperation in practice by displaying a professional andapproachable demeanor
  
• Completesall medical record documentation daily, working with aquality-based coder to optimize comprehensive documentation
  
• Followspolicy and protocol defined by Clinical Leadership
  
• Meetswith Lead Physician about quality of care, review of outcome data,and medical records issues
  
• Clinicallyfocused on direct patient care, inclusive of patient-facing timeand general administrative time (charting, meetings, ) as itrelates to direct patient care
  

PrimaryCare Physician Required - Qualifications:

• Graduateof accredited MD or DO program of an accredited university
  
• Currentand unrestricted medical license or willing to obtain a medicallicense in the state of practice; eligible and willing to obtainlicenses in other states in the region of assignment, as required
  
• BoardCertification or Eligible to become certified (ABMS or AOA) inFamily Medicine, Internal Medicine or Geriatric Medicine preferred
  
• Activeand unrestricted DEA license
  
• MedicareProvider Number
  
• MedicaidProvider Number
  
• Demonstratea high level of skill with interpersonal relationships andcommunications with colleagues/patients
  
• Fullyengaged in the concept of team-based care model
  
• Willingnessand ability to learn/adapt to practice in a value-based caresetting
  
• Superiorpatient/customer service
  
• Basiccomputer skills, including email and EMR
  

PrimaryCare Physician Preferred - Qualifications:

• Knowledgeof HEDIS/STARS
  
• Knowledgeof CAHPS and HOS
  
• Experienceworking in medically underserved/culturally diverse communities
  
• Experienceworking with an electronic health record
  
• Experienceworking in Medicare Advantage Global Risk Models
