JOB DESCRIPTION

Duties And Responsibilities :

• Assess the status of the patient's health problems and determine whether the prescribed medications are optimally meeting the patient's needs and goals of care.
• Evaluate the appropriateness and effectiveness of the patient's medications.
• Identify high-risk patients and implements interventions to improve quality and safety.
• Complete critical patient monitoring and review patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
• Provide appropriate evidence-based, cost-effective, and patient-centered medication recommendations.
• Follow the patient's progress to determine the response of the patient condition to the treatment and assess the prescribed medications.
• Works as an active member of multidisciplinary team and collaborates with healthcare providers to provide patient-centered care
• Mange and adjust patient' medications based on the desired goal and patient risk factors.
• Prescribe/discontinue/renew medicines that are within their scope of specialty/practice.
• Actively participates in stewardship activities and restricted medication programs.
• Provides discharge medication review, reconciliation, and counseling as appropriate.
• Serve as a drug information resource for the providers and support staff in ASH.
• Utilizes existing computer resources to maximize retrieval of clinical information for monitoring and review of the medication use process.
• Actively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.
• Coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health.
• Identifies and prioritizes opportunities for performance improvement based on findings from ongoing monitoring programs and other resources.
• Serves as a liaison between Pharmacy Department and the hospital Performance Improvement Department.
• Submit detailed monthly report to pharmacy administration included the clinical pharmacist recommendation and other hand response.
• Provide and ensure adequate education/training for the trainees/residents.
• Enrich the training experience of the clinical pharmacy program to enable the trainees to become independent and competent future practitioners.
• Provide a valuable contribution to enhance the clinical pharmacy program.
• Performs other tasks and duties within the scope of the knowledge, skills, and experience required for the job, as assigned by the immediate supervisor.
• Provide personalized coordinated care, support and treatment for patients and families.
• Treat people with dignity, compassion, and respect.
• Involve patients, families and caregivers by providing them with education and resources for offering optimal support to promote recovery.

QUALIFICATIONS :

• Graduate of an ACPE accredited College of Pharmacy.
• Doctor of Pharmacy or other advanced degree highly preferred.
• Completion of PGY-1 Pharmacy Residency.
• Completion of PGY-2 Specialty Pharmacy Residency is preferred.

PROFESSIONAL EXPERIENCE :

• Clinical pharmacy experience (2-4 years) in a hospital setting preferred.
• Minimum of two years of expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments would be preferable.

SPECIALIZED SKILLS :

• Good command of English, Arabic preferred or is highly desirable.
• Significant knowledge in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments
